The Chapter In A Box is a toolkit designed for chapters of the International Society for Performance Improvement. This toolkit can be used by chapters that have been in existence for many years and would like a refresher on the best practices used to run a successful chapter, or by chapters that are just starting up.

Each year, the Chapter Partnership Committee hosts the annual Chapter Leaders Workshop (CLW) during the annual conference. This year, the CLW Team’s theme was growing an outstanding chapter and using the Chapter in Box tool as a new resource to guide this growth. This interactive clinic was designed for anyone with or wishing to have a local ISPI presence. This is an adaption from the original. The placemat is a one-sided Word file. The HPT Model and Code of Ethics are on the left side. Program information is on the right side. The original placemat was 11x17, and this version is legal size. Feel free to convert this to meet your needs. For example, you could move the HPT Model & Code of Ethics to the back and use the front for notes. The document outlines the terms of affiliation, rights of the Chapter, liability and representations, termination of agreement, etc. Chapter Officer Listing Use this form to submit updates to your local chapter officers Model Chapter Bylaws These sample bylaws serve as a starting point for Chapters to develop their own governance structure and rules, also known as bylaws. Some items in these sample bylaws may not be suitable for your own Chapter and changes should be made to best accommodate your local Chapter needs. Roster Template This sample roster template may be used to submit chapter rostered on a quarterly basis as required by the Chapter Affiliation Agreement.
